It is not a cause and offers little, if any, basis for substantive health care interventions. Consistent throughout each chapter is the identification of another major issue for African Americans: access to health care. For many African Americans in the United States, access to health services is not always possible. Nor do African Americans always obtain the care they need. African Americans who are in a higher socioeconomic group may find that health and health care is better for them. Nonetheless, for most African Americans this simply isn't true.
Although the treatment of choice for ESRD patients are compatible kidney transplants, there is a gap in the supply and demand of available kidney donors particularly for African Americans. Multiple reasons for this gap include a shortage of available organs, donor-recipient incompatibility, prolonged waiting time, and the reluctance among African Americans to donate organs. In this chapter, then, I will discuss issues related to the problem of kidney procurement for African American recipients and propose strategies for increasing their participation in the national pool of kidney donors.
Where is the outrage, the dissatisfaction, the hurt? These thoughts constantly and consistently troubled me. Then, too, I was embarrassed because I felt that somehow I was not being absolutely fair to my colleagues. Perhaps I was, in fact, too critical. Perhaps I was searching for positions that need not be present for advances to be made. Perhaps the very essence of being African American in this country, as I conceived it, had been lost. A sense of struggle for health care was dramatically absent.
